A first trimester early pregnancy and dating ultrasound is an examination you have during the first 13 weeks of pregnancy. Due to the size of the developing structures and embryo, it is uncommon to perform an ultrasound before 5 weeks gestation 5 weeks after the start of your last menstrual period. Ultrasound is not consistently able to confirm viability and fetal heart activity prior to 6 weeks gestation, so unless there is an urgent need to have an early pregnancy and dating scan earlier most of these ultrasounds are performed after 6 weeks and ideally at 8 weeks for dating purposes.
Urgent cases may still be performed earlier, please discuss this with your referring doctor. Your ultrasound may be performed transabdominally or transvaginally. Transvaginal ultrasound involves placing a thin transducer slightly thicker than a tampon into the vagina.

A dating scan is an ultrasound examination which is performed in order to establish the gestational age of the pregnancy. Most dating scans are done with a trans-abdominal transducer and a fullish bladder. If the pregnancy is very early the gestation sac and fetus will not be big enough to see, so the transvaginal approach will give better pictures.
Dating scans are usually recommended if there is doubt about the validity of the last menstrual period. By 6 to 7 weeks gestation the fetus is clearly seen on trans-vaginal ultrasound and the heart beat can be seen at this early stage 90 to beats per minute under 6 to 7 weeks, then to beats per minute as the baby matures. Ultrasounds performed during the first 12 weeks of pregnancy are generally within 3 – 5 days of accuracy. The most accurate time is between 8 and 11 weeks gestation.
This is because the fetus is growing so quickly that there is a big difference in size from week to week. However, the accuracy of the ultrasound examination is always dependent on the skill of the sonographer and the quality of the equipment. The EDD from the early dating scan is used – if the last menstrual period is not known or is unreliable, or the dating scan differs from the last menstrual period dating by more than 5 days.

The procedure is a standard part of prenatal care in many countries, as it can provide a variety of information about the health of the mother, the timing and progress of the pregnancy, and the health and development of the embryo or fetus.
The International Society of Ultrasound in Obstetrics and Gynecology ISUOG recommends that pregnant women have routine obstetric ultrasounds between 18 weeks’ and 22 weeks’ gestational age the anatomy scan in order to confirm pregnancy dating, to measure the fetus so that growth abnormalities can be recognized quickly later in pregnancy, and to assess for congenital malformations and multiple pregnancies twins, etc.
Performing an ultrasound at this early stage of pregnancy can more accurately confirm the timing of the pregnancy, and can also assess for multiple fetuses and major congenital abnormalities at an earlier stage. Unless there are concerns about the pregnancy it is better to wait to have this scan after 7 weeks as the images are clearer for you to see and better for us to measure. If you come before 7 weeks the baby will be less than 1cm in size and so the scan sometimes has to be performed transvaginally placing the probe inside the vagina to enable us to see the baby well enough to measure and to see the heartbeat.
We ask that you come with a full bladder for this scan as it usually allows us to get the best possible picture of the baby. If you decide to go ahead with this optional screening test you will be offered a scan between 12 weeks and 13 weeks 6 days ideally 12 weeks. This is the only time that this scan can be performed. We send these measurements to LabPlus in Auckland where a blood sample from you is also sent. LabPlus then calculate the risk or chance that your pregnancy is affected with Down Syndrome.
Because this calculation is not carried out on site we are not able to give you the results straight away on the day.

All women will be offered ultrasound scans of their baby at around 12 and 20 weeks. Ultrasound scanning has been used extensively in pregnancy and is accepted to be safe. This scan also measures the baby and estimates how many weeks pregnant you are. If you are more than 8 weeks pregnant this is an accurate calculation, and will be used to estimate the date your baby is due to be born known as estimated date of delivery or EDD. This is a screening test that checks for possible physical problems with your baby. The test is offered to all women, but not everyone will choose to have it. If you choose not to have this scan your choice will be respected and you will be offered a later pregnancy management scan. Whilst this scan is good at detecting most serious problems, it cannot pick up all possible problems with the baby.
The sonographer will have the screen in a position to give them the best view of the baby.
